Great! We're glad your all ready to get involved with our summer camps! We just need a little information about you and your Junior Golfer so that we can get you setup for our fun and exciting summer camps.

Contract and Consent
EMERGENCY and MEDICAL CONSENT By enrolling my child(ren) in Lost Canyons Golf Club summer camp(s), I hereby authorize the procurement of whatever emergency treatment may be necessary for my child(ren). I also authorize the removal of child(ren) from its premises for the purpose of obtaining such emergency medical treatment if the need arises. I agree to hold Lost Canyons Golf Club harmless for the nature, performance and outcome of any such emergency medical treatment and that the determination of whatever an emergency has arisen shall be left to the sole discretion of Lost Canyons Golf Club. PHOTOGRAPHIC CONSENT By enrolling my child(ren) in Lost Canyons Golf Club summer camp(s), I hereby grant Lost Canyons Golf Club permission to photograph my child(ren) in a reasonable and professional manner for promotional and advertising purposes, (i.e. camp videos, scrapbooks, brochures, golf swing videos, etc.) MEDICAL FEES By enrolling my child(ren) in Lost Canyons Golf Club summer camp(s), I hereby agree to be solely responsible for any fees and cost arising from any medical conditions or treatments for my child(ren) arising from my child(ren)’s participation in Lost Canyons Golf Club summer camp(s). AGGRESSIVE BEHAVIOR By enrolling my child(ren) in Lost Canyons Golf Club summer camp(s), I hereby agree that in order to maintain the safety of all campers, aggressive behavior by child(ren) will not be tolerated.

Rules and Roles
We want each camper to get what he or she wants out of their week at camp. In order for that to happen, campers, coaches, and parents must understand and agree to these rules and roles. Junior Campers • Be ready to go on time • Stay positive • Help others • No trash talking or improper language • Observe course rules and etiquette • Keep everyone safe at all times Coaches & Assistants • Be organized and planned • Communicate ideas clearly • Stay Positive • Enforce rules consistently • Keep everyone safe at all times Parents • Get campers here on time • Positive encouragement • Communicate any concerns to lead coach first • Keep your golf coaching ideas to yourself • Support safety and sportsmanship rules We understand and agree that the participation in this program is based on following safety and sportsmanship guidelines. Serious or repetitive violations of these rules will result in suspension from the program without refund.
